At ABA Advantage, we take the initiative to collect relevant insurance details and engage directly with your insurance provider. We shoulder the responsibility of understanding the details of your benefits and your eligibility in regards to ABA therapy.
Some benefit details we can gain for you are:
- Co-pay/deductible information for ABA
- Minimum/maximum hours of ABA therapy approved
- Requirements for authorizations per your insurance provider
Information we will need from you
- Name of Your Health Insurance Provider
- Name of the Primary Insured on Your Policy
- Policy Number
- Specific Member ID Number
- Primary Insured's Date of Birth
- Name and Date of Birth of the Patient Receiving ABA:
